Job summary

PruittHealth Corporation in York County, SC, seeks an Occupational Therapist for Home Health per diem services. You will evaluate and treat patients in their homes, perform visits, and document care using OASIS and other required tools to guide the plan of care.

You will collaborate with an interdisciplinary team, adjust interventions as needed, and ensure timely reports while maintaining compliance with all regulations.

Qualifications

• Must have a valid and unrestricted professional license in state of practice.
• Graduate of ACOTE accredited school.
• If foreign-trained, the program must be deemed equivalent to a U.S. entry-level OT education by a credentials evaluation organization approved by AOTA.
• Home Health experience is preferable.

Responsibilities

• Assesses, monitors, documents and reports patient health and condition using required documents and clinician technology.
• Collaborates with interdisciplinary team and physician to develop and manage plan of care.
• Delivers treatment interventions appropriate for home health patients.
• Maintains current licenses, certifications, and clinical records in compliance with regulations.
• Completes documentation timely and accurately at the point of care.
