Qualifications

• Graduate of ACOTE accredited school.
• If foreign-trained, credentials must be deemed equivalent by an approved agency.

Responsibilities

• Demonstrates knowledge of home care and competency in patient-care skills.
• Communicate and document tasks promptly and accurately.
• Collaborate with interdisciplinary team to maintain continuity of care.
• Follow procedures for emergencies impacting service delivery.
• Report patient progress to supervisory personnel.
• Ensure clean, safe environment with stocked supplies and functioning equipment.
• Contribute to teamwork and support cultural diversity.
• Report violations to supervisor until resolved.
• Self-directed, prioritize, manage time, meet deadlines.
