Interim HealthCare Inc. is seeking an Occupational Therapist to join our home health team in Portsmouth, NH on a part-time basis. You will deliver personalized in-home therapy to help patients regain independence and improve daily living skills.

Qualifications

• Active NH OT license required.
• Graduate from an accredited OT program.
• Minimum 2 years clinical experience (home health preferred).
• Current CPR certification.
• Strong clinical judgment and communication skills.
• Ability to work independently with a flexible schedule.

Responsibilities

• Provide in-home occupational therapy services to patients, and in assisted living settings when applicable.
• Evaluate patients' physical, cognitive, and environmental needs.
• Develop and implement individualized care plans with the healthcare team.
• Help patients improve daily living skills, mobility, and independence.
• Recommend adaptive equipment and home modifications to enhance safety.
• Educate patients and caregivers on treatment plans, exercises, and long-term wellness strategies.
• Document patient progress and adjust care plans as needed.

What You’ll Do

• Provide in-home occupational therapy services to patients and, when applicable, in assisted living settings

What We’re Looking For
