Job summary

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a full-time role performing home health visits for clients in Concord, NH and surrounding Plymouth area. The PT will join a multidisciplinary team to provide rehabilitative care and skilled nursing, with a focus on helping clients remain at home.

Candidate must hold a NH PT license, have at least one year of post-licensure experience, and be able to work independently.

Qualifications

• New Hampshire PT license required.
• Minimum of one year of recent experience as a Physical Therapist.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements and assess mobility, balance, strength, and safety.
• Develop and implement individualized care plans including manual therapy, gait training, and balance interventions.
• Educate clients and family members on rehabilitative care and activities for independence.
• Document observations and interventions in EMR with a state-of-the-art touch pad tablet.
