Job summary

ABL Health Care seeks a compassionate, organized Occupational Therapist (OT) to join our Home Health team in Pleasanton, CA. This full-time role involves evaluating patients, creating care plans, and delivering skilled OT services in patients’ homes, with collaboration from the Clinical Director and staff.

You will work remotely and in the field, travel within a 15–25 mile radius, and use company devices to document care. CA licensure and Home Health experience are preferred.

Qualifications

• Licensed OT in California required.
• Experience in Home Health or Hospice a plus.
• Ability to travel within a 15–25 mile radius regularly.

Responsibilities

• Provide, coordinate and oversee skilled intermittent care to patients in the home.
• Regularly evaluates and re-evaluates patient status.
• Administer treatments, monitor conditions, and report changes to the clinical team.
• Develop, implement and revise the plan of care (POC).
• Educate patients, families, and caregivers about condition and care needs.
• Maintain accurate records and documentation using company devices.
• Comply with all policies and procedures to ensure high standards of care.
