Job summary

ABL Health Care LLC in Pleasanton, CA is seeking a compassionate and organized Occupational Therapist (OT) for a full-time Home Health role. You will evaluate patients, create and implement individualized care plans, and work with the Clinical Director and team both remotely and in the field to maintain high standards of care.

Responsibilities include regular assessments, implementing the POC, coordinating with family and care teams, and ensuring thorough documentation.

Qualifications

• Current CA OT license required.
• 1 year Home Health or Hospice care experience preferred.
• Valid California driver’s license and ability to travel within a 15–25 mile radius.

Responsibilities

• Provide skilled intermittent care to patients in the home and educate patients and caregivers to promote maximal functional status.
• Regularly evaluate and re-evaluate patient status.
• Administer treatments, monitor conditions, and report changes to the physician or nursing leadership.
• Develop, implement, and revise the patient's plan of care (POC).
• Address problems identified in the POC and update the care plan accordingly.
• Provide educational information to patients and families about the condition and care.
• Communicate with other disciplines involved in the case as warranted.
• Maintain accurate clinical records and submit documentation using company devices.
