Job summary

Puget Sound Hospice is seeking a dedicated Occupational Therapist for full-time home health in Thurston County, WA. Provide one-on-one patient care, assess function, and design individualized OT plans to improve ADLs and safety in home environments.

Qualifications

• Licensed Occupational Therapist in Washington state.
• One to two years of experience as an OT.
• Current BLS certification.

Responsibilities

• Provide comprehensive patient care through functional assessments and development of client-centered OT plans.
• Develop and implement individualized OT care plans focused on ADLs, IADLs, safety and participation.
• Educate patients and families on therapeutic techniques and safe use of adaptive equipment.
• Collaborate with interdisciplinary teams and ensure safe discharge planning.
• Maintain accurate documentation of progress and outcomes.
