Job summary

Puget Sound Home Health & Hospice in Kitsap County, WA is seeking a compassionate Occupational Therapist to join our Home Health team. You’ll help patients regain independence, improve safety, and accomplish daily activities in the comfort of home.

Compensation is $45–$65/hour, based on experience, with mileage reimbursement and travel stipend. We offer comprehensive benefits and a supportive, collaborative environment focused on patient-centered care.

Qualifications

• Current Washington State Occupational Therapist license, or eligible before start.
• 1–2 years OT experience.
• Current CPR certification.
• Ability to work independently while collaborating with an interdisciplinary team.
• Home Health or community-based OT experience preferred.

Responsibilities

• Assess patients’ functional abilities, limitations, and home environment.
• Develop individualized treatment plans focused on independence and meaningful daily activities.
• Provide skilled OT interventions to improve mobility, strength, coordination, safety, and activities of daily living.
• Evaluate home safety and recommend adaptive equipment or modifications.
• Educate patients, families, and caregivers on techniques that support independence and safety.
• Collaborate with nurses, therapists, providers, and other members of the interdisciplinary care team.
• Monitor patient progress and adjust treatment as needs change.
• Complete accurate and timely clinical documentation.
