Job summary

A leading home healthcare provider in Colorado seeks an Occupational Therapist to deliver skilled therapy services in patients' homes. The role requires evaluating patient needs, delivering evidence-based interventions, and collaborating with healthcare professionals to enhance patient care. Qualifications

• Minimum of two years of experience as an OT in an acute or rehabilitation setting. Prior home health experience preferred.
• Current CPR certification; TB screening within the past 12 months and Hepatitis consent/declination must be on file.
• Valid driver’s license, reliable transportation, and current automobile insurance are required.

Responsibilities

• Perform comprehensive occupational therapy evaluations including OASIS assessments.
• Deliver skilled, evidence-based occupational therapy interventions based on patient diagnosis.
• Communicate clinically significant findings to the physician and interdisciplinary team.

Skills

Current, unrestricted State Occupational Therapy license

Experience in acute or rehabilitation setting

Patient confidentiality and HIPAA compliance

Education

Graduation from an accredited Occupational Therapy program
