Aveanna Healthcare in Wisconsin is seeking an Occupational Therapist (OT) to join our home health team. You will assess, teach, and deliver therapy interventions directed by the Physician, and collaborate with an interdisciplinary team to optimize patient well-being.

This full-time position offers a $15,000 sign-on bonus, weekday schedules, and territory coverage in Viroqua, Westby and Coon Valley. You will document outcomes, plan care, and support discharge planning in the patient’s home.

Qualifications

• Degree in Occupational Therapy from an accredited university/program.
• Current, unrestricted state license as an Occupational Therapist in the state of practice.
• Valid CPR.

Responsabilités

• Perform initial and ongoing assessments of mobility, coordination, strength and function.
• Provide evaluation and plan of care defining OT needs, goals and outcomes.
• Deliver OT services per physician orders and state regulations.
• Document performance and modify goals; determine discharge plans as appropriate.
• Discharge planning and education of patient and caretakers on home exercise plan.
• Coordinate services with health care team, physicians, nurses, and family.
• Maintain communication; supervise OTA/HHA as applicable.
• Complete OASIS assessments where allowed by state practice.
