Job summary

Aveanna Healthcare is seeking an Occupational Therapist to join our home health team serving Sioux City and surrounding communities. You will assess, treat, and educate patients under physician direction, coordinating with an interdisciplinary team to optimize patient outcomes.

The role is full-time with visits Monday through Friday, offering a $10,000 sign-on bonus and flexible pay structures. You will document care plans and progress, discharge when appropriate, and support families in home

Qualifications

• Must hold a master’s degree in occupational therapy and be licensed to practice in the state.
• Current CPR certification is required.
• Ability to perform comprehensive home health OT assessments and treatment planning.

Responsibilities

• Perform initial and ongoing assessments of mobility, strength, and functional capacity.
• Develop and document plans of care with measurable outcomes.
• Deliver OT services per physician orders in the home health setting.
• Maintain timely documentation and periodic discharge planning.
• Collaborate with the care team and educate patients and caregivers.
