Job summary

Aveanna Healthcare is seeking a licensed Occupational Therapist to provide home health OT services in New Castle County, DE. This part-time PRN role involves assessing mobility, teaching therapeutic interventions, and delivering care under physician orders in patients' homes.

You will collaborate with an interdisciplinary team, document outcomes, and contribute to discharge planning while following the home health plan of care. Valid state license and CPR are required.

Qualifications

• Degree in Occupational Therapy from an accredited university/program.
• Current, unrestricted state license as an Occupational Therapist in the state of practice.
• Valid CPR.
• Valid Driver's License and Auto Insurance.

Responsibilities

• Perform initial and ongoing assessments to evaluate mobility, coordination, strength and current level of function.
• Provide a written evaluation and plan of care defining OT needs, goals, progress and expected outcomes.
• Delivery of OT services to comply with physician orders and home health plan of care.
• Maintain plan of care with timely documentation and discharge planning as appropriate.
• Coordinate services with health care team and educate patient and caregivers on home exercise plan.
