Aveanna Healthcare in Newark, Delaware is seeking an Occupational Therapy Assistant to join our home health care team on a part-time visit basis. The OTA will deliver direct therapy under an occupational therapist, update families on care plans, and help maintain patient safety and function in daily activities.

The ideal candidate has completed an OTA program, holds a current state license, and demonstrates strong documentation and communication skills to support patient progress and caregiver

Formación

• Graduation from an Occupational Therapy Assistant program.
• Current, unrestricted state license as an OTA.
• Demonstrated proficiency in clinical assessments, documentation and compliance with policies.

Responsabilidades

• Provides direct occupational therapy according to directions of occupational therapist and plan of care.
• Communicates with patient and caregiver regarding the plan of care.
• Provides skilled therapy services, treatments and procedures as appropriate for patient care and safety.
• Records clinical notes and progress in a timely manner to ensure continuity of care.
• Plans discharge instructions and communicates discharge plan with patients and caregivers.
• Collaborates with other service disciplines to discuss the patient plan of care.
