Job summary

LiveWell Homecare Agency is seeking a compassionate Home Health Occupational Therapist to deliver personalized therapy in patients' homes across New Jersey. You will assess daily functioning, set goals, and document progress within a multidisciplinary team, emphasizing safety and independence.

Qualifications

• Graduate of an accredited Occupational Therapy program with an active NJ OT license.
• Minimum of 2 years of experience, preferably in home healthcare.
• CPR certification, knowledge of home health regulations, strong clinical judgment, interpersonal skills, and compassion.

Responsibilities

• Assess patients, establish therapy goals, and document progress while working as part of a multidisciplinary team.
• Provide exercises to enhance fine motor skills and coordination, and suggest adaptive equipment to support independence.
• Evaluate fall risks and develop strategies to improve home safety.
• Educate patients and families on care plans, exercises, and self-care techniques.
