Job summary

Ameriwell Care, a leading support agency in New Jersey, seeks a compassionate Occupational Therapist for our Home Health Visits program. You will work directly with individuals in their homes to provide personalized therapy, helping clients reach goals and improve well-being.

Responsibilities include conducting home visits, creating individualized plans, collaborating with families and healthcare teams, using evidence-based techniques, and maintaining thorough documentation.

Qualifications

• Master's degree in Occupational Therapy (OT) or related field.
• Current licensure as an Occupational Therapist (OT) in the state of New Jersey.

Responsibilities

• Conduct home-based occupational therapy visits to evaluate and treat individuals with developmental disabilities.
• Develop and implement individualized treatment plans tailored to each client's needs and goals.
• Collaborate with multidisciplinary teams, including family members, caregivers, and healthcare providers, to ensure comprehensive care.
• Utilize evidence-based therapy techniques to promote client independence, mobility, and participation in daily activities.
• Maintain accurate and timely documentation of client progress, treatment plans, and billing information.
