Qualifications

• Active OT license in New York required.
• Ability to work independently in home health.
• Strong clinical, communication, and time-management skills.
• Reliable transportation required.

Responsibilities

• Evaluate and treat geriatric patients in their homes throughout the Bronx.
• Perform occupational therapy evaluations and develop individualized plans of care.
• Provide one-on-one treatments based on patient needs and goals.
• Manage treatment plans with clinical autonomy and accurate documentation.
• Improve independence, safety, mobility, and activities of daily living.
• Communicate with patients, families, physicians, and the clinical team.
• Coordinate with scheduling, patient care, and logistics.
