PurposeCare is seeking an Occupational Therapist to deliver skilled visits in patients’ homes, focusing on meaningful, patient-centered care. You will create personalized care plans, track progress, and collaborate with families and the care team to meet goals.

This role offers autonomy, flexible scheduling, and the opportunity to work with a compassionate team using innovative home health technologies. Indiana licensure and clean background are required.

Qualifications

• Current Occupational Therapist license in the state you are applying.
• 1 year of OT experience preferred; home health and OASIS experience encouraged.
• Ability to perform physical tasks including bending and lifting up to 50 lbs.
• Valid driver’s license and reliable transportation access.
• Ability to successfully complete a background check.

Responsibilities

• Provide skilled occupational therapy visits in patients' homes.
