PurposeCare is seeking a licensed Occupational Therapist to join our home health team, delivering skilled visits in patients' homes. You will assess, plan, and implement therapy focused on real-life recovery and independence.

Qualifications

• Current Occupational Therapist license in the state you are applying
• 1 year of occupational therapist experience preferred — home health and OASIS experience preferred
• Ability to perform physical tasks including lifting up to 50 lbs, bending and standing
• Access to dependable transportation and ability to complete a background check

Responsibilities

• Provide skilled home visits as an Occupational Therapist
• Assess and address the full scope of a patient’s needs in their home
• Collaborate with patients to build meaningful, one-on-one care plans
• Make clinical decisions autonomously to support patient recovery
