Cantex Continuing Care Network is seeking a licensed Occupational Therapist to provide comprehensive OT services in the home health setting. You will evaluate patients, develop plans of care, and coordinate with the interdisciplinary team to improve functional independence and safety during the home health episode and discharge.

Responsibilities include timely evaluations, documentation, re-evaluations, and caregiver education, with emphasis on quality outcomes and regulatory compliance.

Formación

• Licensed Occupational Therapist per state requirements.
• Home health experience preferred.
• CPR certification preferred.

Responsabilidades

• Screen, evaluate, and treat patients receiving OT services.
• Complete OT evaluations within three working days of Start of Care.
• Coordinate plan of care with interdisciplinary team and physicians.
• Document visits, interventions, and progress in chart per regulations.
• Perform re-evaluations and modify plans of care as needed.
• Support discharge planning and caregiver education.
