Job summary

Qualifications

• Graduate of an accredited school of Occupational Therapy.
• Valid New Hampshire OT license to practice.
• Required life support certification per policy; BLS/ACLS or Heartsaver CPR AED as applicable.
• Minimum of one year of acute care experience; home health preferred.
• Strong communication, documentation, organization, time management and interdisciplinary collaboration.

Responsibilities

• Perform comprehensive OT assessments and develop plans of care.
• Provide skilled OT interventions, prevention, wellness, and client education in home.
• Educate clients and caregivers on safety, home exercise programs and equipment use.
• Monitor outcomes and communicate progress to physicians and team members.
• Coordinate care delivery, participate in case conferences and ensure compliant documentation.
• Supervise Home Health Aides as required by law and policy.
