Job summary

Qualifications

Responsibilities

• Perform comprehensive occupational therapy assessments and evaluations in home health.
• Develop and update individualized plans of care based on assessment findings and physician orders.
• Provide skilled occupational therapy interventions and client education to support safe function in the home.
• Educate clients and caregivers regarding safety, home exercise programs, and follow-up care.
• Evaluate client outcomes, monitor response to treatment, and communicate progress to physicians and team members.
• Coordinates care and participates in case conferences as needed to support quality client care.
• Ensures timely, accurate, and compliant documentation of visits and services.
• Supervises Home Health Aides in accordance with regulations and agency policy.
