Job summary

Peace and Harmony Homecare LLC seeks an Occupational Therapist to provide professional home-based OT services aligned with care plans. You will collaborate with the RN case manager, physicians, and health care team to ensure safe, effective therapy in patients' homes.

Responsibilities include intake, home assessments, developing plans of care, and ongoing documentation. Requires MA licensure, ACOTE-accredited education, and CPR certification. Some travel to patient residences is expected.

Qualifications

• Graduated from ACOTE-accredited OT program and eligible for NBOT certification.
• Licensed OT in Massachusetts.
• Minimum one year OT experience, preferably home care.
• Criminal background check completed and CPR current.
• Foreign-trained OTs must meet US-equivalency requirements.

Responsibilities

• Receive intake referral information and prepare OT paperwork/tools for visits.
• Perform home safety checks and environmental assessments.
• Conduct OT evaluation and document findings.
• Develop and submit Plan of Care to physicians for approval.
• Assess ADL performance and assist patient function level.
• Coordinate with RN case manager to provide services.
• Review billing with patient/family and discuss coverage.
• Supervise Home Health Aide per regulations and needs.
