Job summary

Optum's Mederi Caretenders home health team is seeking an Occupational Therapist focused on assessment and planning for patients in their homes. You'll evaluate functional status, daily living activities, fine motor skills, and adaptive equipment needs, coordinating with physicians and the care team.

The role emphasizes evidence-based practice, CMS documentation readiness, and ongoing communication with nurses and physicians.

Qualifications

• Active Occupational Therapy license in the practicing state.
• Current CPR certification.
• Ability to travel to home health visits as needed.
• Ability to function in any home situation regardless of client demographics.

Responsibilities

• Provides service within the scope of practice as defined by state laws governing the practice of Occupational Therapy, in accordance with the plan of care, using evidence-based techniques, and in coordination with other members of the health care team.
• Evaluates the patient's functional status, status of all body systems as required for CMS documentation, and occupational therapy needs. Consults with the physician in the development of the therapy plan of care.
• Observes, records, and reports to the nurse supervisor and the physician the patient's response to treatment and changes to the patient's condition.
• Conducts patient assessments evaluating the level of function by applying diagnostic and prognostic functional ability tests.
