Job summary

Mederi Caretenders, part of the UnitedHealth/LHC Group family, seeks an Occupational Therapist for home health. You will assess and address patients’ functional status, ADLs, fine motor skills, home safety, and adaptive equipment needs as defined by medical conditions.

Responsibilities include evaluating functional status, coordinating with physicians, and documenting patient progress, with opportunities for growth within a supportive team environment.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Provides service within the scope of practice as defined by the state laws governing Occupational Therapy, in accordance with the plan of care, using evidence-based techniques, and in coordination with other members of the health care team.
• Evaluates the patient's functional status, status of all body systems as required for CMS documentation, and occupational therapy needs. Consults with the physician in the development of the therapy plan of care
• Observes, records, and reports to the nurse supervisor and the physician the patient's response to treatment and changes to the patient's condition
• Conducts patient assessments evaluating the level of function by applying diagnostic and prognostic functional ability tests
