Job summary

Andwell Health Partners in Maine seeks a licensed Occupational Therapist to provide inpatient and community-based OT services in clients’ homes. The role emphasizes autonomous, patient-centered care, discharge planning, and collaboration with families and physicians to meet outcomes.

You will develop individual plans, educate clients on rights and responsibilities, and coordinate resources to promote independence and safety in daily living within home settings.

Qualifications

• Master’s or Doctorate in Occupational Therapy from an accredited institution.
• One-year clinical OT experience preferred.
• Home Health experience preferred.
• Current OT license in State of Maine and CPR card.

Responsibilities

• Develops occupational therapy plan of care with measurable outcomes in collaboration with client/family and physician.
• Provides education and promotes client’s rights and responsibilities in the plan of care.
• Develops, implements, and revises a plan of care aligned with client age and needs.
• Implements care activities toward independent living for client/family.
• Assesses needs for occupational interventions and evaluates effectiveness.
• Refers to internal/external resources to meet client outcomes.
• Initiates procedures to support client safety and care planning.
• Evaluates progress and coordinates discharge planning with client, family and health professionals.
