Job summary

Andwell Health Partners seeks a licensed Occupational Therapist to deliver home health services in Augusta, ME. You will develop and monitor an OT plan of care, educate clients and families, and work toward discharge goals with physician collaboration.

We value independent practice, strong clinical judgment, and the ability to navigate community resources. A current Maine OT license and CPR certification are required.

Qualifications

• Master's or Doctorate in Occupational Therapy required or preferred.
• Current OT license in Maine and CPR certification.
• Home health OT experience is a plus.

Responsibilities

• Develops OT plan of care with measurable outcomes with client/family and physician.
• Provides education about client's rights and plan of care.
• Implements and revises care plan for independence at home.
• Assess needs and evaluate effectiveness of occupational interventions.
• Coordinate discharge planning with client, family, and health professionals.
