Optum Louisiana HomeCare of Covington is seeking a dedicated Occupational Therapist to assess and plan patient care in a home health setting. You will evaluate activities of daily living, functional status, and need for adaptive equipment, collaborating with physicians to develop effective therapy plans.

This full-time role emphasizes high-quality care, strong communication, and independent work within a supportive team that values Caring, Connecting, and Growing together.

Competenze

• Current Occupational Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.
• Demonstrated ability to manage multiple tasks simultaneously.
• Demonstrated ability to work independently.
• Good communication, writing, and organizational skills.

Mansioni

• Provides service within the scope of practice as defined by state laws governing Occupational Therapy, per plan of care with the health care team.
• Evaluates functional status and CMS documentation needs; consults with physician on the therapy plan of care.
• Observes, records and reports patient responses to treatment and changes in condition.
• Conducts patient assessments evaluating functional ability using diagnostic/prognostic tests.
• Function in any home setting regardless of patient demographics.
