Louisiana HomeCare of Covington, part of the LHC Group, seeks an Occupational Therapist to assess and plan home health care focusing on functional status, activities of daily living, and adaptive equipment. You will coordinate with physicians and nurses to develop the care plan and document patient progress.

This role supports visits in the home, offering a mix of direct patient care and productivity targets, with compensation based on per-visit points and hourly rates.

Competenze

• Current Occupational Therapy licensure in state of practice.
• Current driver's license and access to a dependable vehicle or public transportation.
• CPR certification preferred or able to complete within 90 days of hire.

Mansioni

• Provides service within the scope of practice as defined by state laws and the plan of care, coordinating with the health care team.
• Evaluates patient functional status and CMS-required data; consults with physician on care plan.
• Observes, records, and reports patient response to treatment and changes to condition to the nurse and physician.
• Conducts assessments of functional level using diagnostic and prognostic tests.
• Function in diverse home settings regardless of age, race, creed, color, sex, disability, or financial condition.
