Job summary

Commonwealth Home Health in Kentucky is seeking an experienced Occupational Therapist to join our home health team. You will assess and address functional status, activities of daily living, fine motor skills, and adaptive equipment needs within the patient’s home setting.

In this role, you’ll collaborate with physicians and the care team to develop and implement plans of care, document progress, and ensure compliant, high-quality care for diverse patients.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Valid driver's license and reliable transportation.
• CPR certification or ability to obtain within 90 days of hire.
• Ability to manage multiple tasks simultaneously.
• Ability to work independently.
• Good communication, writing, and organizational skills.

Responsibilities

• Provides service within the scope of Occupational Therapy as defined by state laws governing the practice of Occupational Therapy, in accordance with the plan of care, using evidence-based techniques, and in coordination with other members of the health care team
• Evaluates the patient's functional status, status of all body systems as required for CMS documentation, and occupational therapy needs. Consults with the physician in the development of the therapy plan of care
• Observes, records, and reports to the nurse supervisor and the physician the patient's response to treatment and changes to the patient's condition
• Conducts patient assessments evaluating the level of function by applying diagnostic and prognostic functional ability tests
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
