Job summary

Caretenders, part of the LHC Group within the Optum family, seeks an Occupational Therapist in Home Health to assess and address functional status, ADLs, fine motor skills, home safety, and adaptive equipment needs.

Responsibilities include practicing per state OT laws, developing care plans with physicians, documenting CMS requirements, and reporting patient progress to the care team. Salary reflects regional pay scales and benefits.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current CPR certification.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of client demographics.

Responsibilities

• Provides OT services within scope of practice per state laws and plan of care.
• Evaluates functional status and CMS-required documentation; consults with physician for care plan.
• Observes, records, and reports patient progress to nurse supervisor and physician.
• Conducts patient assessments evaluating function using appropriate tests.
