Job summary

Caretenders, a part of LHC Group within the Optum family, is seeking an Occupational Therapist in Home Health. The role focuses on assessing functional status, ADLs, and home safety to develop individualized therapy plans that support patients' independence.

You will work within a team of clinicians to monitor progress, adjust plans, and ensure high-quality patient care in home settings across the community.

Qualifications

• OT licensure required.
• CPR certification required.
• Driver's license or reliable transportation preferred.
• Ability to work with diverse patient populations.

Responsibilities

• Assess and evaluate patient care needs related to ADLs and functional status.
• Collaborate with physicians to develop the therapy plan of care.
• Monitor patient responses and report changes to the care team.
• Perform home assessments and recommend adaptive equipment.
• Document diagnostic and prognostic functional assessments.
