Job summary

Freedom Home Health and Hospice Care Services Inc. in Yuba City, CA is seeking a Certified Occupational Therapy Assistant to support the implementation of therapy plans under the supervision of a registered OT.

The role focuses on functional assessments, home environment safety, and activities of daily living to promote independent living. The COTA will maintain clinical records, participate in team inservices, and coordinate care with other health professionals.

Qualifications

• Graduate of an occupational therapy assistant curriculum accredited by the American Occupational Therapy Association.
• Certified with the National Certification Examination (NBCOT).
• Licensed by the State Board of Healing Arts in accordance with state licensure laws.

Responsibilities

• Assist in ongoing evaluation of patient functional status under OT supervision.
• Teach and retrain patients in daily activities and ADLs.
• Maintain clinical records and report changes to the OT.
• Follow treatment plans and document patient progress and responses.
• Coordinate with care team to ensure integrated, efficient care.
