Job summary

CARE FIRST HOME HEALTH CARE INC in California seeks a Certified Occupational Therapy Assistant – Home Health to support therapy services in patients’ homes under supervision of a Registered Occupational Therapist. The role focuses on implementing standards of care and executing delegated treatment plans to promote independence.

Responsibilities include evaluating functional status, teaching daily living activities, maintaining records, and coordinating with the care team.

Qualifications

• Graduate of an occupational therapy assistant curriculum.
• Licensed by the State Board.
• Current CPR certification is required.
• Must have own insured vehicle meeting state/organizational requirements.

Responsibilities

• Participates in ongoing evaluation of patient functional status as delegated by the OT.
• Teaches new skills or retrains patients in daily activities.
• Maintains clinical records and reports changes to the OT.
• Documents treatment progress and patient responses in the clinical record.
• Coordinates with other health care personnel to ensure coordinated care.
• Instructs family/caregivers on treatment regimes and home care needs.
