Job summary

Bridge Home Health and Hospice seeks a Certified Occupational Therapy Assistant to support the Occupational Therapist in home health care. Position assists with patient prep, assessments, and therapeutic treatments in a home setting.

Candidate must hold Ohio OTA licensure, maintain agency standards, and possess strong communication skills and valid driver’s license to serve patients across the life span.

Qualifications

• Current Occupational Therapy Assistant license in Ohio
• Understands agency mission and regulatory standards
• Valid driver’s license and BVHS fleet policy awareness
• Strong interpersonal and communication skills
• Ability to assess patient status across age groups

Responsibilities

• Assist OT in preparing patients and equipment for treatments.
• Support the therapist in patient assessments and care planning.
• Maintain timely verbal and written reports to RN or OT.
• Apply therapeutic principles within the scope of an assistant.
• Document in line with Bridge Home Health and Hospice guidelines.
• Manage assignments and inform management of incidents.
• Use a team approach for family education and patient care.
• Deliver care within budget constraints while maintaining quality.
• Demonstrate service excellence aligned with organizational values.
• Comply with policies and Medicare/Medicaid requirements.
