Job summary

Boost Home Healthcare is seeking a dedicated Occupational Therapist to provide skilled OT services in patient homes, promoting independence and quality of life. You will assess, plan, implement, and reassess care with a patient-centered approach.

Requirements include a Graduate OT curriculum, state licensure, and NBCOT/National Registration credentials, with acute care experience and home health preferred. Excellent communication and field support are valued.

Qualifications

• Graduate of an Occupational Therapy curriculum accredited by the AMA/AOTA and/or NBCOT certification.
• Licensed by the state Board of Occupational and Physical Therapy Examiners.
• Minimum of one year in an acute care setting.
• One year Home Health experience preferred.

Responsibilities

• Improve or minimize residual physical disabilities of the patient.
• Return the individual to optimum and productive living within the patient’s capability.
• Periodically participate with all other home care personnel in patient’s care planning.
• Provide prescribed occupational therapy and all skilled procedures.
• Initial assessment, plan of care maintenance program development and modifications with 30-day reassessments.
• Consult with physicians regarding changes in treatment.
