Job summary

Harmonyhomehealth is seeking a Home Health Occupational Therapy Assistant to join our team, performing therapist-guided visits in patients' homes under an OT plan of care. You will document in the EMR and collaborate closely with the care team to optimize patient outcomes.

The role requires an active OTA license, CPR/BLS, a valid driver’s license, and strong organizational skills to manage in-home assessments and interventions.

Qualifications

• Active State Certified Occupational Therapy Assistant License is required.
• Current CPR with BLS card is required.
• Valid driver’s license and auto insurance are required.
• Knowledge of Federal, State, and local home health laws and guidelines.

Responsibilities

• Provide OT services under a plan of care established by the OT.
• Deliver direct, evidence-based interventions in patients' homes.
• Communicate effectively with patients, families, and co-workers.
