Job summary

Boost Home Healthcare - Northern Colorado seeks an Occupational Therapist to deliver skilled therapy in patient homes, focusing on improving function and independence. You will assess, plan, implement, and reassess a patient’s care in collaboration with a multidisciplinary team.

The role requires licensure and eligibility for national certification, with 1 year of acute care experience and Home Health preferred.

Qualifications

• Graduate of an Occupational Therapy program and eligible for the national certification exam.
• Licensed by the state Board of Occupational and Physical Therapy Examiners.
• Minimum 1 year in an acute care setting; 1 year Home Health preferred.

Responsibilities

• Improve or minimize residual physical disabilities of the patient.
• Return the patient to optimum and productive living within the patient’s capability.
• Participate in care planning with other home care personnel.
• Provide prescribed occupational therapy and all skilled procedures.
• Initial assessment, plan of care maintenance program development and reassessments every 30 days.
• Consult with physicians regarding changes in treatment.
