Job summary

Harmony Home Health & Hospice in Utah seeks a Home Health Occupational Therapy Assistant to deliver OT services in patients' homes, following an established plan of care and working under OT supervision. The role emphasizes patient-centered care and flexible, scheduling-friendly hours.

The OTA will perform direct interventions, communicate with patients and staff, and document progress using an EMR system. A valid OTA license and CPR/BLS are required, with a driver’s license and reliable vehicle

Qualifications

Active State Certified Occupational Therapy Assistant License is required.
Current CPR with BLS certification is required.
Valid driver’s license and auto insurance are needed.
Knowledge of federal, state and local home health regulations is important.
Strong organization and ability to work independently are essential.
Ability to stay calm in emergencies or crises is required.
Experience with Electronic Medical Records (EMR) systems is preferred.

Responsibilities

Provide physician-prescribed occupational therapy under a plan of care established by the OT.
Deliver evidence-based direct intervention with patients in their home.
Communicate effectively with a diverse patient population and co-workers.
Work under supervision of an OT, including sharing updates on patient status and treatment changes.
Submit required documentation in a timely manner.
