Job summary

Oasis Health Partners is seeking an Occupational Therapist to direct, supervise, evaluate, and provide therapy services to home-based clients. The role includes guiding patients, coordinating visits, and ensuring proper documentation of progress and outcomes.

The candidate must have OT education, state licensure or registration where required, and relevant clinical experience. Travel to client residences and collaboration with the care team are essential components of the position.

Qualifications

• Graduated from an OT program approved by recognized bodies.
• Registered with the American Occupational Therapy Association or equivalent.
• At least 2 years of institutional experience.
• Licensed and/or registered by the applicable state, if required.

Responsibilities

• Evaluates and sets goals based on current functioning and improvement potential, with re-evaluations as needed.
• Instructs clients in prescribed activities to improve independence.
• Establishes household management routines.
• Documents client progress and therapy responses for the provider.
• Instructs clients and families in use of prosthetic/orthotic devices and assistive devices.
• Completes OT discharge summaries within policy frameworks.
• Communicates with care team and coordinates visits with scheduling.
