Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Provides service within the scope of practice as defined by state laws governing Occupational Therapy and the plan of care.
• Evaluates the patient’s functional status and CMS-relevant data; consults with physician to develop the therapy plan of care.
• Observes, records, and reports patient responses to treatment and changes in condition to the nurse supervisor and physician.
• Conducts patient assessments evaluating functional level using appropriate tests.
• Functionally adapt to home environments regardless of age, race, creed, color, sex, or financial condition.
