Job summary

BAYADA Home Health Care in Delaware is seeking an experienced Occupational Therapist (OT) for full-time home health visits in New Castle County. You will evaluate patients, develop individualized treatment plans, and collaborate with a multidisciplinary team to support safe, independent living at home.

The role requires an active DE OT license, strong clinical skills, and the ability to manage visits in a geographic territory.

Qualifications

• Active DE Occupational Therapist license.
• Minimum of one year of recent OT experience.
• Home health experience preferred but not required.
• Graduation from an OT program accredited by AMA/AOTA.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of patients' functional status and ADL/IADL, endurance, and visual coordination.
• Assess cognitive, psychological and motor aspects and develop individualized intervention plans.
• Evaluate home environment for hazards and barriers to independence.
• Identify equipment needs and environmental adaptations.
• Collaborate with nursing and other disciplines in care planning.
• Educate patients and families on rehabilitative care and activities to promote health and independence.
