Job summary

BAYADA Home Health Care is seeking an experienced Occupational Therapist (OT) for a full-time role providing home health visits in the Newark, Delaware area. The OT collaborates with a multidisciplinary team to deliver rehabilitative care, assesses living environments, and educates families to promote health and independence.

Qualified candidates have a DE OT license, at least one year of OT experience, and strong time-management and communication skills.

Qualifications

• Current Delaware Occupational Therapist license.
• Minimum one year of recent OT experience.
• OT program graduation from AMA/AOTA-accredited school.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of patients' functional status including ADL/IADL.
• Assess cognitive, psychological and motor aspects and develop intervention plans.
• Evaluate home environment for hazards or barriers to independence.
• Identify equipment needs and environmental adaptations for better outcomes.
• Revise OT care plans and participate in multidisciplinary care planning.
• Educate patients and families in rehabilitative care and activities.
• Document observations and interventions using an EMR tablet.
