BAYADA Home Health Care in the Delaware Valley seeks an experienced Occupational Therapist to provide home health visits across Delaware County and parts of Philadelphia. This full-time role supports adults and seniors in their homes, delivering rehabilitative care within a multidisciplinary team.

You will assess functional status, design intervention plans, ensure safe home environments, and educate clients and families.

Competenze

• PA Occupational Therapist license a must.
• Graduation from an OT program accredited by AMA/AOTA.
• Minimum one year OT experience; home care preferred.
• Ability to work independently and manage time.
• Strong interpersonal, organizational and problem-solving skills.
• Solid computer skills; EMR experience preferred.

Mansioni

• Make home visits to clients in designated geographic territories.
• Evaluate functional status, including ADL/IADL performance.
• Assess cognitive, psychological and motor aspects to plan care.
• Evaluate home environment for hazards and independence barriers.
• Identify equipment needs and environmental adaptations.
• Update and revise OT care plans with the multidisciplinary team.
• Educate patients and families in rehabilitative care and activities.
• Document observations and interventions using an EMR touchpad tablet.
