Job summary

Qualifications

• Must have active Occupational Therapy licensure in the state of practice.
• CPR certification current.
• Driver license and access to reliable vehicle or public transportation.

Responsibilities

• Assesses patient care needs related to functional status and activities of daily living.
• Evaluates CMS-required functional status and coordinates with physician to develop plan of care.
• Observes and reports patient responses to treatment and changes to condition.
• Conducts patient assessments of functional ability using appropriate tests.
