Job summary

VitalCaring Group is seeking an Occupational Therapist (OT) for Home Health in Oklahoma. This full-time field-based position emphasizes patient-centered care, collaboration with families and interdisciplinary teams, and delivering high-impact home health services.

You will evaluate functional status, plan personalized treatments, and oversee therapy plans while ensuring safe transitions for patients at home.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current NBCOT certification.
• Current state licensure as an Occupational Therapist.
• Minimum of one year of clinical experience preferred (home health experience a plus).
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate patients’ functional status, home mobility, strength, coordination, communication, and daily activity performance.
• Assess home environments to identify safety risks and recommend modifications or adaptive equipment.
• Develop individualized treatment plans with measurable goals to improve independence and QoL.
• Provide therapeutic interventions to restore function and retrain in daily living activities.
• Fabricate splints and train patients in adaptive, prosthetic, and orthotic device use as appropriate.
• Support patients returning to work through functional/work capacity evaluations.
• Educate patients, families, and caregivers on plans, safety, and ongoing care needs.
• Collaborate with the care team to coordinate services and ensure patient-centered care.
• Participate in discharge planning to support transitions and continued progress.
