VitalCaring Group in Texas is seeking a dedicated Occupational Therapist to deliver home health services. As a full-time OT in a field-based role, you will assess patients, develop tailored treatment plans, and collaborate with families and the care team to support independence at home.

Applicants should be a graduate of an accredited OT program with NBCOT certification and current state licensure; CPR, a valid driver’s license, and reliable transportation are required.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current NBCOT certification.
• Current state OT licensure.
• CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate patients’ functional status and home safety.
• Develop individualized home-based treatment plans with measurable goals.
• Provide interventions to restore function and independence.
• Fabricate splints and train use of assistive devices as appropriate.
• Educate patients and families on plans, safety, and ongoing care needs.
• Collaborate with care team for seamless patient-centered care.
• Participate in discharge planning and transitions.
