Job summary

Beth Israel Lahey Health is seeking an Occupational Therapist to provide home health services across the Gloucester, Rockport, and Manchester areas. The role focuses on community health education and direct patient care in patients' homes.

Responsibilities include assessing patients, developing plans of care with families, supervising COTA staff, coordinating with physicians and payers, and supporting discharge planning within MA regulations.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Licensed and certified Occupational Therapist with MA licensure.
• Active BLS certification.

Responsibilities

• Coordinate patient care for a caseload, supervising COTA staff.
• Complete initial patient assessment and develop a care plan with family input.
• Initiate and revise care plans as patient status evolves.
• Provide preventive and rehabilitative therapy and administer prescribed treatments.
• Counsel patient and family, align with cultural values and goals.
• Instruct and supervise home health aides, ensure care quality.
• Coordinate with physicians, payers, and other providers.
