Job summary

BAYADA Home Health Care is seeking an experienced Occupational Therapist (OT) for home health visits in Northumberland County. Per diem, part-time, or full-time options are available to support adult and geriatric patients in their homes.

As a key member of a multidisciplinary team, you will evaluate function, safety, and independence, plan individualized interventions, and educate patients and families to promote health and safe living at home.

Qualifications

• Pennsylvania OT license required.
• Minimum one year of OT experience.
• Graduation from an OT program accredited by AMA/AOTA.
• Ability to work independently and manage time.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of patients’ functional status and ADL/IADL performance.
• Assess cognitive, psychological, and motor aspects and develop plans.
• Evaluate home environment for hazards and barriers.
• Identify equipment needs and environmental adaptations.
• Educate patients and families on rehabilitative care and safety.
• Document observations and interventions, using EMR.
