Job summary

BAYADA Home Health Care is seeking an experienced Occupational Therapist (OT) for Home Health Visits in Chester County, PA. This full-time, part-time or per diem role offers a chance to work with adults and geriatric patients in their homes, Monday–Friday 8:30am–5pm.

Prior home health experience is preferred but not required, with comprehensive training and PPE provided. You will perform evaluations, develop care plans, educate patients and families, and document care using a touch pad EMR

Qualifications

Current Pennsylvania Occupational Therapist license required.
Prior home health experience preferred but not required.
Minimum of one year of recent OT work experience.

Responsibilities

Make home visits to clients in designated geographic territories.
Perform thorough evaluations of patients’ functional status and ADL/IADL performance.
Assess cognitive, psychological and motor aspects and develop intervention plans.
Evaluate home environment for hazards or barriers to independence.
Identify equipment needs and environmental adaptations.
Continually assess and revise the OT care plan with the multidisciplinary team.
Educate patients and family members in rehabilitative care to promote safety and independence.
Document observations and interventions in the EMR system.
