Job summary

BAYADA Home Health Care seeks an experienced Occupational Therapist to provide skilled home health services in Lehigh County. The OT, including OT, evaluates clients in their homes, develops care plans, and collaborates with a multidisciplinary team to support safe, independent living.

This role offers per visit compensation, with options for per diem, part-time or full-time employment, serving adult and geriatric clients in a home setting. Pennsylvania licensure is required.

Qualifications

• Licensed Occupational Therapist in Pennsylvania.
• Minimum 1 year recent OT experience.
• Experience in home health is preferred but not required.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Evaluate functional status, ADLs/IADLs, cognitive and motor performance.
• Develop individualized intervention plans and adjust as needed.
• Assess home environment for hazards and independence barriers.
• Educate patients and families on rehabilitative care and safety.
